Responsibility of the Applicant

Responsibilities of applicants:

• To complete the  Application  and supply all information requested.
• When requested, provide a copy of a written estimate or itemized bill from their veterinarian.
• The estimate or bill should be on the veterinarian's letterhead and should include the following:
  1. Name of attending veterinarian and facility
  2. Applicant's name
  3. Dog's name
     
  4. Diagnosis
  5. Prognosis
  6. Complete breakdown of costs
  7. Note: SCAF staff will request that applicant or veterinarian submit via e mail
